Do you live in Toronto? Are You Going to Toronto Police Services To Secure Your PRC/VSS for Fall 2018?

This message is being sent to all students hoping to enter or continue their studies, in our B.Ed Professional Programs (at Lakehead University - Orillia, Heritage Place) in Fall 2018.

All B.Ed students are required to obtain a PRC/VSS (any time after June 15, 2018) for their start/return to B.Ed studies in Fall 2018.


That being said, where your permanent address is, determines where you will go to secure the requested documents.


A previous communication was recently circulated with details and instruction for all students going to a Local Police Service or an OPP detachment in their hometown.


The process for those who are going to Toronto Police Services is different. Please read below for details.


Students should only be visiting a Toronto Police Service location to secure their PRC/VSS (any time after June 15th, 2018) only IF their permanent address is in Toronto.


If you live anywhere else other than Toronto - You are to go to your Police Service location in your hometown. DO NOT USE THIS APPLICATION FORM. 


Please Note: Toronto Police Services can average wait times in excess of 12 to 16 weeks.


Steps You Should Take To Secure Your PRC/VSS From Toronto Police Services:

1) Print the form/application on LEGAL SIZED paper
2) Fill out the form/application
  • Mail it to the address provided on the form along with the accepted forms of payment (check their website for the exact fee and accepted methods of payment).
  • Use a money order or a certified cheque.
  • You may also deliver the form in person and then you can pay by cash, debit or credit card.
3) You submit the completed application (in person or by mail) to:

Caution:
If you choose to mail in your application for PRC/VSS to Toronto Police Services, you do so knowing and accepting the risk that it may delay the processing time and your ability to meet Lakehead's deadline for submitting this required documentation, to be permitted to head out on placement in Fall 2018.

Going To A Local Police Service Location OR Ontario Provincial Police (OPP) For Your PRC/VSS in time for Fall 2018?

This message applies to all students who will be entering or continuing in our B.Ed Professional Programs in Fall 2018 who do not live in the Greater Toronto Area (GTA) and are going anywhere but to Toronto Police Services to secure their required documentation for the 2018-2019 academic year.

As mentioned previously, Toronto Police Services has a specific process for those who live in Toronto, to secure their PRC/VSS.  Please see my upcoming separate communication for the complete details and the specific form that must be used.

The requirement of Local Police Service or Ontario Provincial Police (OPP)It is very likely that your local Police Service or OPP station in your hometown will require a letter from you that explains why it is you need a Police Record Check (PRC) and Vulnerable Sector Screening (VSS).

Attached to this email, you will find a letter that you can print and take with you after June 15th, 2018 to complete your application and start the process of securing your required PRC/VSS document or report for the 2018-2019 academic year.

Starting this process after June 15th, 2018 ensure that your documentation will be valid for the full duration of your Fall 2018 placement.

Remember:
  • Ask if a section of the Application Form (provided at the detachment) needs to be completed by Lakehead University.
    • Do they require an ink signature? If so, make arrangements to bring the application to Julie Howell (jhowell@lakeheadu.ca) to complete. You will have to plan to come in person to the Heritage Place Office (Mon-Fri 8:30am -1:00pm and 2:00 - 4:30pm).
    • If they will accept a fax or scanned signature, simply email the Application Form to jhowell@lakeheadu.ca. She will print the form, complete the Lakehead portion of the form, and arrange to scan or fax the form back to you so you can take it to the police service location, for processing
  • Consider asking for two originals of the report
  • Obtain a receipt
  • Inquire about expected turnaround time 
  • If you receive notification that you must submit to fingerprinting - notify us and start the process immediately
  • Once you have the results, be sure to put it in a safe place
  • Come prepared on the first day of class with both the Original Report and a Photocopy of the PRC/VSS results in 2 sealed envelopes. Please bring them to the Education Administration Office at Heritage Place.

B.Ed Professional Programs - Opportunity for Students to Submit Requests For Same Cohort/ Schedule Assignment For 2018-2019 School Year

Student Requests For Same Cohort/Schedule Assignment
ALL B.Ed Professional Programs (1 Year Program, Year 1 & Year 2 - of the 2 Year Program)
2018-2019 Academic Year
As you prepare to enter Lakehead University's B.Ed Professional Programs, it is important to note that the delivery model for this program is structured in such a way that you are assigned to a "cohort" or one of the possible class schedules with classes running Monday to Friday.  This is done to streamline the delivery of all required academic content that is inherent to your specific B.Ed program of study.

Proactive Requests For Same "Cohort" or Class Schedule Assignment:
In an effort to be more "environmentally conscientious" we are willing to consider individual requests for students to have the same full cohort schedule assignment, for the express purpose of commuter carpooling.
  • The Education Administration Team reserves the right to approve or reject any request
  • Once a decision has been made regarding your request, you will receive an email response.
  • The Education Office will respond to each cohort assignment request no later than May 25th, 2018
  • Please DO NOT contact the Education Office for an update on the status of your request
  • Please complete the survey found later in this email, to have your request considered.
Important Notes To Consider Before You Submit A Carpooling/Same Cohort Request:
  • We have defined a "carpool cohort assignment request" as one not to exceed 2 students in total. (1 driver + 1 passenger) 
  • BOTH students who make up a "carpool cohort request" must complete the survey, for the request to be considered.
  • Naming the SAME STUDENT in multiple carpool cohort requests - will result in all requests mentioning the student's name to be null and void.
  • No requests received/forwarded after May 17th, 2018 will be considered.
  • In order for your request to be considered, it must involve 2 students in the SAME PROGRAM and SAME YEAR. 
BOTTOM LINE:
We need one submission from the DRIVER in the duo, naming the PASSENGER and then another submission made by the PASSENGER in the duo, naming the DRIVER, to have your submission be considered as complete.

Submitting Your Formative (Self Assessment) For March/April 2018 Placement


Good Day B.Ed Students,

This is a reminder to all of you that you should have your self-assessment formative evaluation completed online through the Education Portal, between Wednesday, April 11, 2018 and Friday, April 13th, 2018.

We ask that self-assessments (completed by teacher candidates) and formative assessments (completed by Associate Teachers), not be submitted late in the day on Friday April 13th, because IT assistance will not be available late in the afternoon, should you experience any issues with your submission that require assistance.

Remember:
You are not to submit your self -assessment until your meeting with the AT has occurred and the growth plan has been developed and appropriate notes have been added to the AT Formative assessment.

First, create a draft in Word, then copy your prepared text into the online form, when you are ready to submit.   You meet with your associate to go over your assessment and the learning plan is generated.

Remember:
You are not to submit your self -assessment until your meeting with the AT has occurred and the growth plan has been developed and appropriate notes have been added to the AT Formative assessment.

Step #1
  •  Associate Teacher completes Formative Assessment
  • Teacher Candidate completes their own Self-Assessment

IMPORTANT:
Be sure to save the assessment, but do not click final submit at this point, until both your AT and yourself have had a chance to meet to discuss.

Step #2
Your AT and yourself will meet to discuss the Formative Assessment (prepared by the Associate Teacher) and the Self-Assessment (prepared by the Teacher Candidate) and set learning/growth plan based on discussion points.

Please Note:
  • The Growth Plan is included on the Associate Teacher's version of the  online assessment form
  • Your AT submits their formative evaluation form online through the Education Portal, as well
  • The evaluations will be sent automatically to your Faculty Advisor
  • The link to your self-assessment is:  https://fieldexperience.lakeheadu.ca/candidate/

REMEMBER:
Your Associate Teacher (AT) has been provided with their own weblink with username and password for the Education Portal, to submit their assessment of you.  

All Associate Teachers should have received their password/login/instructions by email.
  • If your AT has not received their login credentials for the Portal, please ask them to email Julie Howell (jhowell@lakeheadu.ca). 
  • If they require a password reset, they can and select "forgot password" on the Portal homepage to reset it, or send a reset request email  to: Julie Howell (jhowell@lakeheadu.ca).
